Job Description We are seeking a Board-Certified or Board-Eligible Primary Care Provider (Internal Medicine or Family Medicine) for a 13 week locum assignment at the VA in Canandaigua, NY. This outpatient role follows the VA’s Patient Aligned Care Team (PACT) model, providing coordinated, team-based care to Veterans in a structured, weekday-only schedule. Assignment Highlights: Rate: $130/hour Schedule: Monday–Friday, 8:00 AM – 4:30 PM Setting: 100% outpatient (no weekends, no federal holidays, no call) Travel and lodging provided Malpractice coverage included Clinical Responsibilities: Adult primary care (panel ~1,200–1,400 patients) 12–15 patients per day Chronic disease management (HTN, DM, COPD, CHF, hyperlipidemia) Preventive screenings and women’s health services Collaborative team-based care with RN, LPN, and Medical Support Assistant Documentation in VA CPRS Qualifications: BC/BE in Internal Medicine or Family Medicine Active, unrestricted U.S. medical license DEA registration, BLS & ACLS certified Eligible for VA credentialing and background clearance This is an excellent opportunity for providers seeking predictable hours, a structured outpatient environment, and strong clinical support.
