Job Description Job Description Position Overview: Pediatric Nurse Practitioner (PNP) or Physician Assistant (PA) needed for an FQHC in Hornell, NY Schedule: Monday: 8:00 AM - 5:30 PM Tuesday - Thursday: 8:00 AM - 8:00 PM Friday: 8:00 AM - 5:00 PM Flexible schedule with 36 hour work week You will join a collaborative team, working with two other Advanced Practice Providers (APPs), and partner with MD/DO in delivering comprehensive, patient-centered care. Responsibilities: Conduct History of Present Illness (HPI) assessments and physical examinations. Diagnose and develop individualized treatment plans for patients. Prescribe medications per New York State statutes and professional guidelines. Perform minor surgical procedures. Provide counseling and education to patients regarding their treatment plans. Generate referrals for specialized care as needed. Contribute to medical decision-making and support the Care Team.
