Job Description Job Description Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (LTC) Location: Youngsville, PA Assignment Length: 8 Weeks Assignment Details Shift Type: Variable Shifts Schedule: 12-hour shifts Days per Week: 3 Total Hours per Week: 36 Guaranteed Hours: 48 hours Pay Rate: $38 to $42/hour Position Overview "Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) setting in Youngsville, PA. This role involves providing high-quality nursing care to residents in a skilled nursing facility while collaborating closely with Registered Nurses, physicians, and interdisciplinary care teams. Responsibilities Provide direct nursing care to residents in a long-term care environment Administer medications and treatments in accordance with physician orders Monitor, assess, and document residents’ conditions; promptly report any changes Assist in developing and implementing resident care plans Maintain accurate and timely medical records Support residents with activities of daily living (ADLs) as needed Ensure compliance with facility policies, safety standards, and state regulations Communicate effectively with residents, families, and healthcare team members Qualifications Active Pennsylvania LPN license (required) Minimum 1 year of LPN experience , preferably in Long-Term Care or Skilled Nursing BLS certification (required) Strong clinical judgment, organizational, and documentation skills Ability to work variable shifts and 12-hour schedules Compassionate, patient-centered approach to care For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
