Job Title: Level 2 Registered Nurse Case Manager Profession: RN
Case Manager Specialty: Case Management Duration: 13 weeks Shift:
Monday to Friday Hours per Shift: 8 hours Experience: Minimum of 1
year as a clinical nurse in a hospital and 9 months in home care
preferred License: Current RN License (New York only)
Certifications: BLS Must-Have: Certified Home Health Care
experience Description: Develop and revise treatment and discharge
plans in conjunction with patients, families, physicians, and the
care delivery team, while obtaining necessary medical orders and
approvals. Complete and submit OASIS assessments, reassessments,
transfers, resumptions of care, discharges, and significant changes
in condition within defined time frames. Process orders and notify
physicians of patient needs and changes in condition. Provide an
environment that promotes respect for patients, their privacy,
confidentiality, and property. Initiate appropriate preventive and
rehabilitative strategies. Deliver services requiring substantial
specialized nursing skills. Consult with and educate patients,
families, and team members regarding disease processes, self-care
techniques, and preventive strategies. Supervise LPNs to ensure
patient health, safety, and compliance with the plan of care.
Direct, coordinate, evaluate, and supervise the quality of patient
care services provided. Interact with patients, physicians, and
referral sources to maintain positive relationships. Support care
management philosophy through collaborative, customer-focused
planning and case management to meet individual health and service
needs. Assure compliance with state and federal legislation through
accurate clinical record maintenance. Maintain current knowledge of
available community-based services. Demonstrate flexibility and
willingness to visit patients based on need. Ensure problem
resolution at the point of contact. Attend patient care conferences
for assigned patients. Monitor results and progress towards
operational and financial objectives. Understand the impact of RN
functions on patient care and provider relationships. Efforts to
reduce avoidable costs and errors are pursued diligently.
Participate in peer review and quality assessment and performance
improvement initiatives. Ensure documentation reflects medical
necessity for care in home settings. Promote a respectful and
diverse workplace environment. Participate in education and
training programs as needed. Demonstrate commitment to professional
