Job Description Note: Min of 3 years of clinical or home health
experience required. Must be licensed and reside in Texas Licensed
Vocational Nurse (LVN/LPN) – Care Coordinator Onsite We are looking
for a Licensed Vocational Nurse (LVN/LPN) to join our team as a
Care Coordinator . In this role, you will support patients and
providers through telephonic outreach, care coordination, and
quality improvement initiatives. What you’ll do: Make outbound
calls to patients to close care gaps and support preventive care
Assist with scheduling screenings, follow-ups, and appointments
Coordinate with primary care providers and healthcare teams Review
reports to identify quality gaps and support value-based care goals
Educate patients and families on health management and medication
adherence Support high-risk patients with multiple health
conditions Focus: Improving patient outcomes, quality performance,
and continuity of care.
