Required Experience: 1 year Medical/Surgical and/or Community Nursing, and prior experience in acute care discharge planning experience. Familiarity with discharge planning/UR process and community agencies. EPIC experience required. 2 years experience required for non U.S. candidates. Preferred Experience: Certification for PRI, Screen and MDS preferred. Required Licenses and Certifications: Active NY RN license. AHA BLS. Contract Duration: 13 weeks.
