ExperienceHourly pay range: $23 - $26Under the direction of an RN,
you will assist with the monitoring and coordination of the
delivery of healthcare services with a manageable caseload for
people with developmental disabilities who are in a structured day
program or living in community base homes. As a human services
agency we are seeking candidates with strong leadership skills,
compassion and the ability to work in a collaborative
environment.In a Residential setting, assists the RN to provide
medical case management for individuals with developmental
disabilities who are living in community-based homes.In a Family
Care setting, assists the RN in working with people living with
certified caregivers and supporting individuals in the caregivers'
home.In a Day Service setting, assist the RN to ensure that nursing
services are provided for those individuals needing services during
the hours of program attendance.In a Respite setting, assists the
RN to provide nursing oversight to people respiting and staff
supporting them in a respite group home setting.ESSENTIAL
FUNCTIONS/RESPONSIBILITIES: Assists with monitoring and
coordinating the delivery of preventive routine health care,
therapies and programs for individuals with developmental
disabilities Assists with ensuring adequate completion of health
related exams, appointments, laboratory/diagnostic testing as
ordered or required. Assists with the monitoring of medication
administration in the facility. Administers medications, treatments
and supplements as assigned. Assists the RN with providing health
services policy training for staff. Attends medical appointments at
the direction of the RN. Participates as an active member of the
Interdisciplinary Team. Maintains compliance with and assists with
ensuring facility compliance with all Physician, NP, PA, etc.
orders, applicable State/Federal regulations, and agency policy and
procedure.MINIMUM QUALIFICATIONS: Current NYS licensure to practice
as a Licensed Practical Nurse Valid Driver's License that meets
agency policy Ability to meet physical requirements of the position
when performing the following: Standing, walking, sitting, bending,
stooping, squatting, kneeling and climbing Lifting requirements of
35lbs Physical agility and ability to react to emergency
situations, including maintaining Strategies for Crisis
Intervention and Prevention (SCIP-R) certification as per
regulatory guidelines CPR certification requiredWhy People
